Back in the Swing’s role in post-treatment care
Now, as Survivorship Care Plans are given to breast cancer survivors, it is time for Back in the Swing to play a new supportive role, to help survivors actualize those Care Plans, to help protect and improve their health 365 days a year. So, in collaboration with the Breast Cancer Survivorship Center at the University of Kansas Cancer Center, and community practices across Greater Kansas City, we will begin the Live Your Plan Project . At the time of the Survivorship Care Plan appointment, each patient will receive, along with their Survivorship Care Plan, The Back in the Swing Cookbook: Recipes for Eating and Living Well Every Day After Breast Cancer by Barbara Unell and Judith Fertig.
Thanks to funding through Back in the Swing, hundreds of breast cancer survivors will receive this on-of-a-kind, award-winning resource to help them actualize their Survivorship Care Plans. The goal is to roll out the program throughout the Greater Kansas City Area, through the Breast Cancer Survivorship Center and the University of Kansas Cancer Center’s ten community practices. In total, these providers care for the majority of the breast cancer patients in our community.
